QI On Tap: Quality Improvement Onsite Technical Assistance Program

QI On-TAP is a program that was created to provide technical assistance in the area public health accreditation readiness including performance management, quality improvement, strategic planning, community health improvement planning, and workforce development.

Consulting Services for 2016 County Health Rankings Release

The goals of this project are to: 1. Work with state?wide and community partners to identify a county within the lower half of the rankings that is in the community health improvement; 2. Provide one?day of onsite training on successful community engagement strategies for taking focused actions for improvement related to prioritized health issues; 3. Offer follow?up technical assistance and review via email communications and phone conferences to complete the selected quality improvement project.

QI On-Tap for Yolo County

PHI is working with the Yolo County Department of Health Services on accreditation, performance measurement and quality improvement activities. The project is developing a standardized approach to agency-wide performance measurement to track program operations in the context of linked community health outcomes and build workforce capacity to enable the department to independently conduct quality improvement projects.
