Santa Cruz County Community Health Improvement

QI On-TAP assisted the County of Santa Cruz with its community health improvement process using the Mobilizing for Action through Planning and Partnerships (MAPP). The products of this work include the Santa Cruz County Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP). The CHA is available on the county's website, and the CHIP will be coming out in late April or early May 2018.

Tulare County CHA and CHIP

QI On-TAP assisted the County of Tulare to conduct a community health improvement planning process using Mobilizing for Action through Planning and Partnerships (MAPP). The products of this work include the Tulare County Community Health Assessment (CHA) and the Tulare County Community Health Improvement Plan (CHIP), and can be accessed here.
