Ventura County Community Information Exchange

The Ventura County Community Information Exchange (VCCIE) is an integrated system of care that promotes cross-sector partnerships, client and resource data sharing, and real-time care coordination to improve population health. The VCCIE system supports an individual or family as they progress through the Ventura County social service delivery system toward their goals and better quality of life. Using a collaborative approach to care between county health and social service providers, shared language, common outcome measures, evaluation and data analysis, the CIE breaks down barriers and reveals resource and funding gaps.
