Workbook for Caregivers: A Supportive Guide

This workbook, created by PHI’s AC Care Alliance, is designed to help caregivers organize caregiving tasks and take care of themselves, featuring worksheets with examples and easy-to-follow instructions.

Caregivers play a critical role in the lives of persons with chronic, serious, and advanced illnesses, often supporting their care recipient with a variety of tasks. They take on not only the burden of additional physical tasks and time commitments to support them, but also the oftentimes emotional burden of caring for someone they love and the uncertainty that comes with illness.

The Workbook for Caregivers, created by PHI’s AC Care Alliance (ACCA), is designed to help caregivers organize caregiving tasks, build a fellowship of people and organizations that can help, plan for difficult conversations that may occur, and take good care of themselves. The Workbook is meant to be approachable and illustrative, featuring personal worksheets, examples and easy-to-follow instructions.

PHI’s ACCA helps improve care for people with advanced illness, serving predominately communities of color throughout California, while supporting caregivers by addressing their caregiving and personal needs. Their work has long recognized the important role that caregivers play in the lives of individuals with chronic, serious, and advanced illnesses. This is especially true for unpaid family caregivers who often regularly support their person needing care and face uncertainty in decision-making and in what will come next.

ACCA aims to ensure caregivers have the support they need to better help their person needing care, and also support their own personal needs, physical health, and well-being, and remind caregivers that they are never alone in this caregiving journey.

In this video, ACCA’s Care Team shares how to use the Workbook for Caregivers. Learn how useful the Workbook can be with examples from AICP participants and the Care Team’s personal experiences. While the Workbook was designed to be used together with a Care Navigator in the AICP, it can also be used on its own.
